Agreement and Waiver
In accordance with California’s 2002 alternative health care law (Senate Bill SB 577), which provides legal protection for complementary and alternative health care practitioners, such as Family Constellations practitioners, the following information is provided:
Liza Miron is not licensed as a healing arts practitioner by the state of California. Liza Miron agrees to provide professional services in accordance with acquired training and experience giving undivided attention during scheduled consultations to facilitate clients' or participants’ benefits. Services utilize the framework and techniques of Systemic Family Constellations, Life and Professional Coaching, Neuro Linguistic Programming and Emotional Freedom Release Technique (Tapping).
Services to be provided do not include the practice of medicine, as Liza Miron is not a licensed physician. These services are non-diagnostic, and are complementary to the healing arts services that are licensed by the state. The California State Legislature has determined that state licensing may not be conferred upon an occupational group for purposes of status or prestige. The primary purpose of licensing laws for legally defined Healing Arts and Mental Health professionals is to protect public health and safety. Accordingly, Family Constellations are not issued licenses by any state governmental agency to engage in their professional services.
As a client or participant, you assume responsibility for your personal safety, conduct and well-being. You understand that this work can involve physical and emotional activities that may be challenging. You also understand that this work is not meant to replace mental or physical health therapy. If you are seeking medical or mental health services, you will consult a licensed professional outside the context of the work you do with Liza Miron.
You, on behalf of your heirs, guardians, and legal representatives, do hereby remit, release, and forever discharge Liza Miron from any claims, suits, demands or actions arising from or growing out of your participation in this work.
Privacy and trust among the group members are fundamental for Constellations. For that reason, by agreeing to participate in the group events, you commit to keeping all personal information that others shared disclosed during the sessions as confidential.
